Hyperuricemia and Inflammatory Reaction:A Study of Their Roles in Pathogenesis of NAFLD

【Abstract】 Objective To investigate the roles of hyperuricemia and inflammatory reaction in the pathogenesis of nonalcoholic fatty liver disease（NAFLD）.Methods A total of 70 NAFLD patients（NAFLD group）diagnosed in this hospital from April to July 2010 were divided，based on uric acid(UA) levels，into subgroups A（n=38，UA≤416 μmol/L），B（n=32，UA＞416 μmol/L），and 74 normal subjects enrolled as control group.Height，weight，waist circumference（WC），hip circumference（HC），blood pressure（BP）were measured，waist-hip ratio（WHR），body mass index（BMI）calculated；alanine aminotransferase（ALT），UA，fasting blood glucose（FBG），glycated hemoglobin（HbA1c），triglyceride（TG），total cholesterol（TC），low density lipoprotein cholesterol（LDL-C），high density lipoprotein cholesterol（HDL-C），fasting insulin（FINS），hyaluronic acid（HA），Ⅳ collagen,IL-18,resistin determined and insulin resistance index（HOMA-IR）calculated.Results Systolic blood pressure（SBP），BMI，WHR，UA，TG，HOMA-IR，ALT，HA，IL-18，resistin were higher in NAFLD group than in control（P＜0.05）.SBP，UA，TG，ALT，IL-18，resistin higher in subgroup B than in subgroup A，HDL-C lower（P＜0.05）.By Spearman correlation analysis，UA was positively correlated with WHR（r=0.331），BMI（r=0.241），ALT（r=0.323），HOMA-IR（r=0.264），TG（r=0.213），resistin（r=0.315），IL-18（r=0.369），negatively with HDL-C（r=-0.224）in NAFLD patients（P＜0.05）；by partial correlation analysis after excluding WHR，BMI，HOMA-IR，TG and HDL-C，UA was positively correlated with ALT（r=0.222），resistin（r=0.326），IL-18（r=0.435）（P＜0.05）；further excluding resistin and IL-18，UA had no linear correlation with ALT（r=0.172，P=0.106）.Conclusion NAFLD patients may predispose to hyperuricemia，hyperuricemia may be involved in the pathogenesis of NAFLD through insulin resistance，inflammation reaction and so on.

The Expression of Leptin/PI3-K/PKB in Patients with Non-alcoholic Fatty Liver Disease

【Abstract】 Objective To evaluate the expression of leptin,leptin receptor (OB-R) and the components in phosphatidylinositol 3-kinase/protein kinase B (PI3-K/PKB) signaling pathway in patients with non-alcoholic fatty liver disease (NAFLD).Methods Totally 30 patients with definite diagnosis of NAFLD treated in our hospital between May 2007 and September 2007 were included in this study,and 30 patients with cholecystic polypus but normal liver treated in our hospital were taken as the control group.The method of immunohistochemstry were used to assess the expression of hepatic leptin,OB-R,PI3-K (p85α) and PKB in all participants,and serum leptin level was determined by radioimmunoassay.This study also analyzed whether the expression of these proteins was correlated with each others.Results The serum leptin level in NAFLD group were significantly higher than that in the control group〔(10.90±3.39)μg/L vs.(5.55±1.96)μg/L,t=-7.841,P＜0.05〕.Compared to the control group,the expression of hepatic leptin,OB-R and PI3-K was also significant higher in NAFLD group (P＜0.05),while the expression of PKB was significantly lower in NAFLD group (P＜0.05).In NAFLD group,the expression of hepatic leptin was positively correlated with serum leptin level and the expression of PI3-K(p85α) (r=0.413，P＜0.05;r=0.365，P＜0.05);While the expression of PKB was negatively correlated with serum leptin level and the expression of PI3-K(p85α)(r=-0.923,P＜0.05;r=-0.854，P＜0.05).In the control group,the expression of hepatic leptin was also positively correlated with serum leptin level and the expression of PI3-K(p85α) (r=0.593,P＜0.05;r=0.795，P＜0.05).But the expression of PKB was positively correlated with serum leptin level and the expression of PI3-K(p85α)(r=0.686，P＜0.05;r=0.840，P＜0.05).Conclusion The expression of hepatic leptin and leptin receptor was high in NAFLD patients,but the expression of PKB was relatively low.Over expression of hepatic leptin in NAFLD patients seems lead to leptin resistance instead of activation of PKB,and the impaired activation of PI3-K by leptin and PI3-K might be a new mechanism of hepatic leptin resistance in NAFLD patients.

Related Influencing Factors of T2DM Combined with NAFLD and Its Relationship to Carotid Artery Atherosclerosis

【Abstract】 Objective To explore the related influencing factors of type 2 diabetes mellitus (T2DM) combined with nonalcoholic fatty liver disease (NAFLD) and its relationship to carotid artery atherosclerosis (CAA).Methods A total of 278 T2DM patients hospitalized in this hospital in 2012 were divided,based on whether combined with NAFLD,into non-NAFLD group (n=152),NAFLD group (n=126).Height,weight,waist circumference (WC),hip circumference (HC),blood pressure (BP),disease duration and other general data were recorded,levels of liver enzymes and glucolipid metabolism detected,and carotid atherosclerotic plaques (CAP) determined.Results No significant difference in gender,age,BP between 2 groups (P＞0.05).The body mass index (BMI),waist-hip ratio (WHR),alanine aminotransferase (LT),aspartate aminotransferase (AST),triglyceride (TG),insulin resistance index (HOMA-IR),carotid intima-media thickness (IMT),incidence of CAA were higher,high density lipoprotein cholesterol (HDL-C) lower in NAFLD group than in non-NAFLD group,the difference was significant (P＜0.05).Partial correlation analysis showed that IMT was positively correlated with HOMA-IR (r=0.368,P＜0.05).Stepwise Logistic regression analysis showed that T2DM combined with NAFLD and HOMA-IR were risk factors of CAA (P＜0.05);BMI,TG,WHR,ALT,HOMA-IR were independent risk factors of NAFLD (P＜0.05).Conclusion Dyslipidemia,insulin resistance and CAA are more vulnerable to T2DM patients combined with NAFLD.NAFLD is related to BMI,TG,WHR,ALT,HOMA-IR.

Nonalcoholic Fatty Liver Disease and SNPs-11377C/G in Promoter Region of Adiponectin Gene:A Correlation Study in Han Elderly Population in Guilin

【Abstract】 Objective To investigate the association of SNPs-11377 C/G in proximal promoter region of adiponectin gene with nonalcoholic fatty liver disease（NAFLD）in Han elderly population in Guilin.Methods From July to November in 2012，height，weight，blood pressure（BP）were measured，body mass index（BMI）calculated，fasting blood glucose（FBG），alanine aminotransferase（ALT），aspartate aminotransferase（AST），triglyceride（TG），total cholesterol（TC），high density lipoprotein cholesterol（HDL-C），low density lipoprotein cholesterol（LDL-C），uric acid（UA）detected in 150 NAFLD patients（NAFLD group）and 130 normal subjects（control group）.NAFLD risk factors were analyzed by univariate Logistic regression and non-conditional Logistic regression analysis，and SNPs-11377 C/G in proximal promoter region of adiponectin gene detected by polymerase chain reaction-restriction fragment length polymorphism（PCR-RFLP）.Genotypes and allele frequency were compared between 2 groups.Results SBP，DBP，FBG and levels of ALT，TG，TC，LDL-C，UA were higher in NAFLD group than in control group（P＜0.05），HDL-C lower（P＜0.01）；No significant difference was noted in AST between 2 groups（P＞0.05）.By univariate Logistic regression analysis，gender，BMI，hypertension，FBG，TG，HDL-C，LDL-C,and UA entered into the regression equation（P＜0.05）；By non-conditional Logistic regression analysis，gender，BMI，TG，UA entered into the regression equation（P＜0.05）.There was no significant difference in SNPs-11377 C/G in proximal promoter region of adiponectin gene or allele frequency between 2 groups（P＞0.05）.There was no significant difference in BMI，SBP，DBP，FBG，ALT，AST，TG，TC，HDL-C，LDL-C,and UA in the NAFLD patients or normal subjects with different genotypes（P＞00.05）.Conclusion There is no obvious correlation between NAFLD and SNPs-11377 C/G in proximal promoter region of adiponectin gene in Han elderly population in Guilin.

Relationship of Prediabetic State to Interaction between Chymase and Traditional Cardiovascular Risk Factors

【Abstract】 Objective To explore the relationship of prediabetic state（PDS）to the interaction between chymase and traditional cardiovascular risk factors，to provide basis for research of diabetes mellitus（DM）mechanism.Methods Fasting blood glucose（FBG），2h postprandial blood glucose（2h PBG）were determined in 1 197 residents aged 55-75 years in urban communities from July to September 2009.Based on the results，71 subjects with normal carbohydrate tolerance（NG group）and 189 PDS subjects（PDS group）were selected to have biochemical and immune parameters detection.The relationship of PDS to the interaction between chymase and traditional cardiovascular risk factors were analyzed.Results FBG，2h PBG，fasting insulin，insulin resistance index（HOMA-IR），systolic blood pressure（SBP），waist circumference（WC），waist-hip ratio（WHR）were higher in PDS group than in NG group，β cell sensitivity index（HOMA-β）lower（P＜0.05）.By univariate Logistic regression analysis，age（60-64 years old），hypertension，central obesity，increased insulin（moderate），decreased HOMA-β and increased HOMA-IR（moderate or high）were positively correlated with PDS，and there was a linear trend between chymase level and varying ages
